Faith Formation Forms

If you have a child that has registered for our 2024/2025 Faith Formation program, please complete the following forms, put them in one envelope with the student’s first & last name printed on the outside, and drop them off in the basket at Mass as soon as possible.

Required Forms for all Faith Formation students

Image Release Form (click to download English, Spanish) | One Form Required per Family
- Please print the form and fill it out
- Make sure the student names are legible
- Sign & date the form
Circle of Grace opt-out forms (one per student, click here to download)
- If you do not want your child(ren) to participate in Circle of Grace, please fill out and sign the opt-out form.

Parent/Guardian Medical Information & Consent Form (one per student, click here to download)
- If you have a child that has a medical condition that we need to be aware of, please fill out the form.
- Please let us know if your child has any special medical or learning needs

Additional Forms for all Confirmation Students

Confirmation Sponsor Form (click here to download)
- The sponsor/proxy must have this form signed by the parish priest of the church they are an active member of.
Confirmation Saint Report (click here to download)
- All students must complete the Confirmation Saint report. The due date will be communicated during Faith Formation class.

Forms for Catechists & Volunteers

All Catechists and volunteers must complete the Safe Environment Training through the Diocese of Orlando, be fingerprinted and pass a background check before volunteering.
