If you have a child that has registered for our 2024/2025 Faith Formation program, please complete the following forms, put them in one envelope with the student’s first & last name printed on the outside, and drop them off in the basket at Mass as soon as possible.

Required Forms for all Faith Formation students
- Image Release Form (click to download English, Spanish) | One Form Required per Family
- Please print the form and fill it out
- Make sure the student names are legible

- Circle of Grace opt-out forms (one per student, click here to download)
- If you do not want your child(ren) to participate in Circle of Grace, please fill out and sign the opt-out form.

Additional Forms for all Confirmation Students

- All students must complete the Confirmation Saint report. The due date will be communicated during Faith Formation class.
Forms for Catechists & Volunteers
- All Catechists and volunteers must complete the Safe Environment Training through the Diocese of Orlando, be fingerprinted and pass a background check before volunteering.
